What is an NGN case study?

Next Generation NCLEX case studies are designed to gather evidence about clinical judgment. A scenario can include information such as nursing notes, vital signs, medication records, laboratory results or provider orders. Items then ask you to work with that information across a sequence.

The official clinical-judgment measurement model names six steps. They are useful for diagnosis after practice: a missed answer may begin with an overlooked cue, an unsupported hypothesis, a scope problem or an evaluation error.

The clinical-judgment sequence

Recognize cues

Identify information that may be clinically meaningful in the scenario and exhibits.

Analyze cues

Connect findings, compare patterns and distinguish what supports or weakens an explanation.

Prioritize hypotheses

Decide which explanation or concern deserves attention first.

Generate solutions

Identify realistic responses that fit the situation, role and available resources.

Take action

Choose the response that best addresses the priority while protecting safety.

Evaluate outcomes

Interpret the client’s response and decide whether the plan worked or needs to change.

Review the pivot point, not just the percentage.

After an NGN practice case, reconstruct the decision path. The most useful question is often “where did my interpretation first separate from the evidence?”

    Write the one-sentence story

    Summarize what is changing and why it matters before reopening explanations.

    Name the decisive cues

    Separate direct evidence from background information and assumptions.

    Find the first reasoning break

    Was the miss caused by recognition, interpretation, priority, action, scope or evaluation?

    Compare every plausible option

    Explain why the selected response fits better—not merely why another answer is “wrong.”

    Transfer one principle

    Create a short rule you could apply to a different client and different wording.
